Checklist for Surgery
 
 


 

SURGERY INSTRUCTIONS

 

All surgical bookings are done through our Surgical Department in the Hamden Office,  Monday – Friday between 9 a.m. – 4 p.m.  If you were seen in one of our satellite offices, the surgical department will contact you within three or four business days.

 

Multiple dates will be given to you.  In order to avoid rescheduling surgery, please consult your schedule prior to choosing a date.    You may have an upcoming commitment (e.g. vacation plans) or need to incorporate surgery with your work schedule. Please notify the surgical coordinators if you have plans to fly in the near future.  It is difficult to reschedule a surgery but if an emergency arises, please call the surgery department AS SOON AS POSSIBLE to give them the information.

 

There is a $250.00 cancellation fee if the department is not given 5 business days notice.  Do not leave the cancellation message with the Answering Service or in the department’s voice mail.  Please speak directly with the surgery department if you must cancel your scheduled surgery.

 

PRE-OPERATIVE REQUIREMENTS

First a surgery date will be determined. Then your medical requirements will be determined and the forms will be mailed to you. Then you must do the following:

____You should schedule an appointment with your Primary Care Physician in order to obtain medical clearance for surgery. This appointment should take place 2-3 weeks before the date of your surgery. Please bring the medical clearance form with you to that appointment. All information should be forwarded directly to our Hamden Office all least three days prior to your surgery date.

____Lab work only.  Please have your lab work done  2-3 weeks prior to your surgery date.  Please check  with your insurance  company regarding network labs.


____It is the patient’s responsibility to see that all requirements for surgery are met.

 

 

 

RESTRICTIONS

Please read and follow the instructions listed below. Failure to do so could cause a postponement of your surgery:

1) Two weeks before your surgery date do not take vitamin E, aspirin, or any aspirin-like products, (example: Motrin, Advil, or Aleve any products containing ibuprofen). You may use Tylenol products. 

Coumadin and Plavix patients must stop these medications a minimum of 5-7 days prior to surgery and 5 days after surgery.

2)  No Neosynephrine or Afrin two weeks prior to surgery

3) Do not have anything to eat or drink after midnight, the night before surgery, not even a sip of water in the morning.

 

4)     Please arrange for a ride to and from the hospital

 

ARRIVAL TIME

 

The facility where your surgery is scheduled will determine and inform you of the time and all the necessary information you need to know for that day. They will be in touch with you one business day prior to your surgery. In the event that you have not heard from them, please call the facility directly. (Phone numbers provided below)

 

We understand that everyone needs to make arrangements for their surgery.  It is important to remember that you are having a surgical procedure and you should plan on devoting the entire day to the needs of your surgery. This office has no control in determining the time of your surgery.  The time is determined by the facility.

POST OPERATIVE CARE

At the hospital, the doctor will give you any necessary prescriptions along with any postoperative instructions. For maximum healing results, please follow the post-operative instructions, and remember to keep all post-operative appointments.

 

SURGICAL BILLING AND INSURANCE INFORMATION

Our office will notify your insurance carrier of the proposed surgery and obtain prior authorization when necessary. Please be aware that it is the patient’s responsibility to comply with the terms of your insurance contract, especially with regards to your deductibles.  If you are aware of any upcoming changes with your insurance company, it is your responsibility to provide us with that updated information. Failure to do so may result in postponement of your surgery.

 

Our office will submit an insurance claim for payment of your surgery.  Please be aware that we will be billing for the doctor’s fee only.   The hospital, anesthesiologist, pathologist and any pre-operative testing are all billed for separately.
