Every plan has its own set of rules. If your plan requires a written referral by your primary care physician, you must bring this with you. If your primary care physician will not provide a referral, and if your plan requires one, you will be responsible for your bill.

If your insurance plan requires paperwork, our staff can help you with the forms. Most of our claims are now sent electronically via computers.

Co-pays and any other required payments are due at the time of service.
